Are You Eligible for Energy Assistance?
General Requirements
The following information will be needed for us to adequately help you. Please bring to your first appointment:
- Social Security Card or legal document indicating name and social security number for all household members
- Picture ID with birth date for applicant
- Verification of total household income (for everyone in the household) for the three months prior to month of appointment
- Adults 18+ with no income in any one of the three months must sign additional forms to confirm the absence of income
- Current month’s home energy bill
- Disconnect notice, if applicable, with current address
- Rent/Lease agreement or proof of residence

If you are physically unable to come into the office for an appointment because of a disability or advanced age, and there is no other able-bodied adult living in your household, we may be able to mail you an application form.
